ECFMG Certification: The Complete Process for International Medical Graduates
By Muntasir Minhaz • Published Aug 19, 2026
ECFMG Certification requires passing USMLE Step 1 and Step 2 CK, completing one of six Certification Pathways that verify your clinical and communication skills, and passing all four OET Medicine sub-tests with no exceptions. For the Match tied to the 2027 cycle, ECFMG's Pathways application is due by January 31, 2027.
🎓 Your medical school must appear in the World Directory of Medical Schools
You need passing scores on USMLE Step 1 and Step 2 Clinical Knowledge
One of six Pathways verifies your clinical and communication skills, based on your license status, your school's OSCE, or six supervised clinical encounters
📩 Every applicant takes the OET Medicine test and must pass all four sub-tests: Listening, Reading, Writing, Speaking
Your medical diploma and final transcript go through ECFMG's EPIC system for verification
ECFMG Certification is what lets you register for the NRMP Match and apply through ERAS
Why ECFMG Certification exists
If you graduated from a medical school outside the United States or Canada, US residency programs cannot accept you into the National Resident Matching Program (NRMP) or the Electronic Residency Application Service (ERAS) until the Educational Commission for Foreign Medical Graduates (ECFMG) certifies you. Certification confirms your medical education, your exam scores, and your clinical and communication skills meet the standard US programs expect.
ECFMG Certification does not replace a state medical license. It is the credential that opens the door to apply for US residency training, the step before licensure.
The four requirements
| Requirement | What it covers |
|---|---|
| Medical school listing | Your school must be listed in the World Directory of Medical Schools at the time you enrolled and graduated |
| USMLE Step 1 | Tests your grasp of basic medical science. Pass or fail only, with no numeric score reported |
| USMLE Step 2 CK | Tests clinical knowledge and its application. You need a passing score |
| Certification Pathway plus OET | Verifies clinical and communication skills through one of six Pathways, plus a passing OET Medicine result |
Full detail on each requirement is on ecfmg.org , which updates the Pathway rules each year.
The six Certification Pathways
ECFMG uses Pathways to verify the clinical and communication skills that a US standardized patient exam would otherwise test. Which Pathway applies to you depends on your license status and your medical school's own exit exams.
Pathway 1: for graduates who currently hold, or recently held, an unrestricted medical license
Pathway 2: for graduates of a school whose national regulatory authority requires an OSCE for licensure, within ECFMG's current eligibility window
Pathways 3 through 5: based on your medical school's accreditation status with an ECFMG-recognized accrediting body
Pathway 6: for graduates who do not qualify for the others, requiring six real, in-person clinical encounters, each evaluated by a licensed physician using the Mini-Clinical Evaluation Exercise (Mini-CEX)
Confirm your eligibility for each Pathway on ECFMG's site before you plan your timeline. Graduation-date and licensure cutoffs change from year to year.
The OET Medicine requirement
Every applicant, regardless of native language or Pathway, must pass the Occupational English Test (OET) Medicine version. You need a satisfactory score across all four sub-tests: Listening, Reading, Writing, and Speaking. ECFMG applies this rule with no exceptions, so a strong score in three sub-tests and a weak one in the fourth still fails the requirement.
Schedule your OET early in your Pathway application. ECFMG sets a final test date each cycle, and a late or failed attempt can push your Certification, and your Match eligibility, into the following year.
Credential verification through EPIC
ECFMG verifies your primary source credentials, your final medical diploma and final medical school transcript, through its online system, EPIC (ECFMG International Credentials Services). You upload your documents and ECFMG contacts your medical school directly to confirm authenticity, a process that takes weeks depending on how quickly your school responds.
Start your EPIC account and request document verification as soon as you decide to pursue US residency, well before you submit your Pathway application. A slow verification from your school is the most common reason applicants miss ECFMG deadlines.
Timeline for the current cycle
ECFMG opens its Pathways application each year in August. For Match eligibility tied to the 2027 cycle, the application deadline is January 31, 2027, Eastern time, and supporting documentation must arrive by the same date, according to ecfmg.org . Pathway 6 carries its own earlier deadline for Mini-CEX evaluations within the cycle, so check the exact date on ECFMG's site rather than assuming it matches the general Pathways deadline.
ECFMG's 2026 Pathways application carried a $925 fee. Confirm the current cycle's fee on ecfmg.org before you budget, since fees are set per cycle.
After you are certified
Once ECFMG certifies you, you register for the NRMP Main Residency Match and apply to residency programs through ERAS using the token ECFMG issues you. Certification alone does not grant a visa or a job. You still need a residency position through the Match, then the visa status, usually J-1 through ECFMG's own sponsorship or H-1B through your program, that lets you train in the US.
Keep your Certification current. Accepted Pathways and the resulting Certificate expire if you do not revalidate them or otherwise meet ECFMG's permanent validation rules, so track your expiration date the same way you track a visa expiration date.
What Certification does not cover
ECFMG Certification does not include USMLE Step 3, the final exam in the USMLE sequence. Most residency programs let you take Step 3 during residency rather than before it, but state medical boards require it before they issue your full license, so check your program's own policy on Step 3 timing.
Certification also does not grant you a US visa or work authorization on its own. It is the credential ECFMG and residency programs require before they process your Match registration or your ECFMG-sponsored J-1 visa, both separate steps.
