Medical School Secondary Applications: What Schools Ask and Why

By Muntasir Minhaz • Published Aug 04, 2026 • Graduate & Professional School

TL;DR

Secondary applications are school-specific essays you complete after AMCAS forwards your verified primary, and most MD and DO programs require one before they consider you for interview.

  • 📩 Secondaries usually ask why you want that specific school, a diversity or adversity essay, and updates since your primary

  • Some schools send a secondary to every applicant automatically, others screen your primary first

  • The 2027 AMCAS primary costs $180 for your first school and $48 for each additional school

  • 💵 Secondary fees vary by school and add up fast when you apply broadly

  • The AAMC Fee Assistance Program cuts MCAT and AMCAS costs, and many schools waive secondary fees for approved recipients

  • ⏱️ Draft your "why this school" and diversity essays before primaries go out, since most prompts repeat cycle to cycle

Medical School Secondary Applications: What Schools Ask and Why

What a secondary application is

After you submit your AMCAS primary application and it clears verification, every medical school you applied to sends you a secondary application. Most MD and DO programs require one, and it typically layers school-specific essays on top of the personal statement and activities list you already submitted through AMCAS.

Some schools send a secondary to every applicant who completes a verified primary, regardless of GPA or MCAT score. Others screen your primary first and only send a secondary to applicants who clear an internal threshold. Check each school's process in the Medical School Admission Requirements (MSAR) database or on its admissions page rather than assuming a secondary means you passed a screen.

Why schools ask for more essays

Your AMCAS primary goes to every school on your list unchanged. Secondaries let admissions committees ask what the primary cannot: why you want their specific program, how you handle adversity, and what has changed in your file since you submitted the primary. Committees use these essays to gauge fit with their mission, curriculum, and location, not to test how much you can write under pressure.

Common secondary prompts

Prompts repeat close to word for word from one cycle to the next at most schools, so you can prepare before your primary even submits. Expect some combination of these:

  • Why this school specifically, tied to curriculum, mission, or location

  • A diversity or adversity essay describing a challenge you overcame or a perspective you bring

  • An update essay covering anything new since you submitted your primary

  • Ties to the state, for public schools that give preference to in-state applicants

  • An optional additional information essay for context that does not fit elsewhere

Search each school's name plus "secondary essay prompts" from the prior cycle to build a first draft bank before your primary goes out. Most schools release the same or lightly revised prompts each year.

What it costs

Application costs stack on top of each other across primary and secondary stages.

CostAmount
AMCAS primary, first school (2027 cycle)$180
AMCAS primary, each additional school$48
Secondary application feeSet by each school, varies
MCAT registration (with Fee Assistance Program)$145

These figures come from the AAMC . Secondary fees are set independently by each school and add up quickly once you apply to 15 or 20 programs, so budget for them separately from your primary application cost.

The AAMC Fee Assistance Program (FAP) helps if your household income falls at or below 400% of the federal poverty level for your family size. For the 2026 cycle, approved applicants get a reduced MCAT fee of $145, a free two-year MSAR subscription, and a waived AMCAS fee covering one application to up to 20 schools, worth more than $2,000 in combined benefits. FAP does not automatically waive secondary fees, but many individual schools waive their own secondary fee for approved FAP recipients, so check each school's policy in MSAR before you pay. The 2026 FAP application opens February 2, 2026.

Turnaround and volume

Once secondaries start arriving, they tend to come in clusters within a few weeks of your primary clearing verification. Track every school's prompts, deadlines, and submission status in one spreadsheet so nothing slips through when several arrive the same week.

Turning a secondary around within one to two weeks signals genuine interest to most committees, even when a school publishes no formal deadline for it. Customize each essay to the specific school rather than sending a generic answer with the school name swapped in. Admissions readers see hundreds of secondaries a cycle and notice copy-paste answers immediately, and a rushed or recycled essay can undercut an otherwise strong application.
