Delivered part-time, this certificate has students join an interprofessional health-care team supporting individuals and families across long-term, community and hospital care settings. Study progresses from foundational engagement to entry-to-practice readiness, covering activities of daily living such as personal care, home management, nutrition and family responsibilities guided by a client's care plan, plus support for clients showing responsive behaviours, facing injury risk or needing end-of-life care. Admission Requirements: This part-time continuing-education program requires an Ontario Secondary School Diploma (OSSD), or equivalent, or applicants 19 years of age or older, plus Grade 12 compulsory English at the C or U level, or Conestoga's Preparatory Communications (COMM1251). The page does not state a standardized English-language test score. Applicants should obtain a clear Vulnerable Sector Screening police check before applying, since access to work-integrated learning placements requires it and students cannot complete the program without the practicum. The program also requires immunizations (MMR, Tdap, varicella, hepatitis B), TB testing, First Aid/CPR certification through Conestoga, and possible seasonal flu and COVID-19 vaccination as directed by placement organizations.
