Overview
The Community Health and Social Medicine (CHASM) Care Worker Program is an Ontario College Certificate offered by the Michener Institute of Education at UHN as a one-year, full-time, in-person program. It is designed for people already working or volunteering in community-facing care roles who bridge hospitals, public health services and community settings, giving them formal credentialing and practical skills for community health work. The curriculum spans two semesters: the first covers an introduction to community health work, physical health and wellbeing, mental health foundations and strategies, service coordination and system navigation, and ethical considerations; the second covers family dynamics in community and individual support, crisis intervention and de-escalation, structural and social determinants of health, Indigenous health and wellbeing, and health education and program development. Admission follows a nomination process run with community and social service organizations rather than open application, and candidates must be trusted, community-rooted care workers committed to equity and access to care. The program is not eligible for OSAP funding; tuition bursaries have been provided for admitted cohorts.
Requirements
Admission Requirements: Admission is by nomination, arranged through community and social service organizations that Michener partners with, rather than a standard open application. Candidates should currently be working or volunteering in a community-facing care role, be trusted by and rooted in the community they serve, be committed to equity, advocacy and improving access to care, and be ready to pursue formal credentialing to support their career. Before starting studies and clinical experiences, admitted students must meet Michener's health requirements, including completing and submitting the Immunization Requirements Form (IRF) by the communicated deadline.
